FAB (For Anything But...Alcohol, drugs or tobacco) 2nd Annual 5 v 5 Basketball Tournament Saturday April 21, 201210AM-5:30PM St. Andrews School 63 Federal Road Barrington RI  $12 per person, may carry up to 7 players on your roster.                                            T-shirts for each player.  Trophies and glory to the winners!

Please print and fill out and return with payment in full to participate, first 16 teams will participate with 2 teams on call.

Payments may be made in person with cash, or by phone with a credit card.

• All participants agree that they intend to participate in the above not for profit sponsored event of their own will and further acknowledge that they do so at their own risk.  The participant agrees to hold harmless and indemnify from litigation or claim resulting from any and all injury or damage to themselves or their personal belongings while participating and while on campus.  The participant will hold harmless and indemnity extends to all persons participating in the event including it's volunteers, organizers, facility personnel and other participants.
